NYHA functional class I & II
4 days postsurgery and hemoglobin ≥ 90 g/L if flight < 2h*. 7 days postsurgery and hemoglobin ≥ 90 g/L if flight ≥ 2h.

* If hemoglobin < 90 g/L and wishes to avoid transfusion, then supplemental oxygen is required.
