Recommendation 16. For patients requiring a switch from clopidogrel to ticagrelor, we recommend a ticagrelor loading dose of 180 mg followed by 90 mg BID, regardless of the timing of the last clopidogrel dose (Strong Recommendation; Moderate-Quality Evidence).
